Structured Health Dialogue

"Health Screening (Approx. 15-20 minutes):~Measurement of height, weight, BMI, and waist-to-hip ratio Blood pressure recording Collection of fasting blood samples to assess glucose, cholesterol, and triglyceride levels~Health and Lifestyle Questionnaire:~Self-reported assessment of physical activity, dietary habits, alcohol and tobacco use, psychosocial stress, and family history of cardiovascular disease~Creation of an Individualized Health Curve:~Visual summary of the patient's current cardiovascular risk factors, showing how these factors interact and where improvement is needed~Structured Health Dialogue (60-90 minutes):~A one-on-one conversation between the participant and a trained nurse using principles of motivational interviewing Focused on identifying personal health goals, enhancing motivation for lifestyle change, and discussing possible support strategies Tailored Health"
